There are lots of ways to get involved and share your views, including (if you're not sure about anything, read our FAQs first here):
You can fill in our survey here...
...or collect a survey from main reception or from an outpatient clinic at any of our hospitals.
You can also request any of this information in languages including Arabic, Bulgarian, Chinese (S), Chinese (T), Czech, Farsi, Hungarian, Kurdish, Polish, Punjabi, Romanian, Somali and Urdu.
It is also available in easy read, large print, audio / braille.
